RNs are the clinical backbone of our agency. As an RN case manager, you open each episode of care: you complete the comprehensive start-of-care assessment (including OASIS documentation for Medicare clients), work with the client’s physician to build an individualized plan of care, and deliver the skilled visits that plan calls for — wound care and dressing changes, medication management, injections and IV therapies, catheter care, and chronic-disease teaching for clients and their families. You supervise the LPNs and home health aides assigned to your clients, reassess progress against care-plan goals at every recertification, and keep the physician and family informed as needs change. Your care plan always has a goal — and you are the one who makes sure we meet it.

Working under the direction of an RN case manager, you deliver hands-on skilled nursing in the client’s home: monitoring vital signs, wound care and dressing changes, administering medications, catheter and ostomy care, and diabetic care. You are often the team member our clients see most, so your observations matter — you watch for changes in condition, report them promptly to the case manager, and document every visit accurately so the whole team can act on what you saw.

The Clinical Director leads day-to-day clinical operations. You supervise our nursing staff, lead case conferences, and review plans of care to make sure every visit matches the physician’s orders and Medicare’s Conditions of Participation. You run our quality assurance and performance improvement (QAPI) activities, guide staff through complex clinical situations, support on-call coverage, and mentor new nurses through orientation. When surveyors visit, your documentation standards are what they see.

The Administrator is responsible for the overall operation of the agency: compliance with Minnesota Department of Health home care licensure and Medicare certification requirements, agency policies and procedures, budgeting, staffing, and survey readiness. You also represent ABC in the community — building relationships with hospitals, clinics, social workers, and county case managers so the people who need home care can find us.

Our Social Worker helps clients and families navigate everything around the care itself. You complete psychosocial assessments, provide short-term counseling and support, help families plan for long-term care decisions, and connect clients to community resources — including Minnesota’s home and community-based waiver programs such as the Elderly Waiver (EW) and Community Access for Disability Inclusion (CADI). You are often the difference between a family coping alone and a family with a plan.

The Administrative Assistant keeps the office — and the schedule — running. You process new referrals and intake paperwork, verify insurance coverage, schedule visits so every client sees the right professional at the right time, maintain medical records, and answer the phone when a family calls with a question. For many clients, your voice is their first impression of ABC.

Provided through our contracted therapy partners, Physical Therapists help clients move safely in their own homes. You evaluate mobility, strength, and balance, design home exercise programs, provide gait training with walkers and other assistive devices, and build fall-prevention plans — so recovering from surgery or managing a chronic condition doesn’t mean leaving home.

Provided through our contracted therapy partners, Occupational Therapists help clients do the everyday things that make life feel like their own — dressing, bathing, cooking, and moving through their home safely. You assess how a client performs activities of daily living, recommend adaptive equipment and home-safety modifications, and teach energy-conservation techniques that protect independence.

Provided through our contracted therapy partners, Speech-Language Pathologists work with clients on communication and swallowing. You evaluate and treat speech, language, voice, and cognitive-communication difficulties — often after a stroke or with a progressive condition — and build safe-swallowing strategies so clients can eat and connect with the people around them.
