Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.

ABC Home Health Care Plus, LLC (“ABC”) is required by law to maintain the privacy of your protected health information, to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

How we may use and disclose your health information

For treatment. We use your health information to provide your home care — for example, our nurses share information with your physician to carry out and update your plan of care.

For payment. We use and disclose your information to bill and collect payment for services — for example, submitting claims to Medicare, Medicaid, or your insurance plan.

For health care operations. We use your information to run our agency and make sure our clients receive quality care — for example, quality reviews and staff supervision.

We may also use or disclose your information: when required by law; for public health activities; to report suspected abuse, neglect, or vulnerable-adult concerns as required by Minnesota law; for health oversight activities such as Minnesota Department of Health surveys; in response to a court order or other lawful process; to coroners, medical examiners, or funeral directors; to avert a serious threat to health or safety; for workers’ compensation purposes; and to family members or others involved in your care when appropriate, unless you object.

Uses that require your written authorization

Most uses and disclosures for marketing purposes, any sale of your health information, and other uses not described in this notice will be made only with your written authorization. You may revoke an authorization at any time, in writing, except where we have already acted on it.

Your rights

  • Inspect and receive a copy of your health record, including an electronic copy
  • Request corrections (amendments) to your record
  • Receive an accounting of certain disclosures we have made
  • Request restrictions on how we use or share your information
  • Request that we communicate with you in a certain way or at a certain location
  • Receive a paper copy of this notice at any time, even if you agreed to receive it electronically
  • Be notified if a breach of your unsecured health information occurs

Changes to this notice

We reserve the right to change this notice and to make the revised notice effective for information we already hold. The current notice is always available on this page and at our office.

You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be penalized or retaliated against for filing a complaint.
