Just about anyone – your doctor, family and friends – even you can refer yourself to ABC Home Health Care Plus.
Social Workers, nursing homes, hospitals, rehab units and insurance companies may also refer you.
How to make a referral
Call us: (612) 326-9377, Monday through Friday. An ABC registered nurse will take the details and schedule the first assessment visit.
Fax physician orders or referral documents: (612) 326-1206.
Email: info@abchomecare.org with your contact information and we will call you back. Please don’t include detailed medical information in email — we’ll gather that securely by phone.
